Every practice has its own mix, but the same handful of forms carries most of the work. Below are the ones that turn up again and again, written out with the official titles so you can match them against the guide without second guessing.
Two rules sit above the list. Work from the checklist for the exact stream, because the same client type can need different forms in different programs. And check the revision date on the day you submit, not on the day you started the file.
